The name of the card is a bit curious... Down, A, B, Up, Up, Right.

#4 could also be Doom II: Hell on Earth, because IDKFA worked there too.
If it were IDCLEV, it would be the original Doom, because Doom II's level warp cheat was different.

Agreed, Sim City it is. (did anyone ever manage to get the big statue of Mario when you built a huge city?! )

The game was Contra. How can anyone forget Contra?
And while the code was featured in virtually every Konami game, I don't know anyone who doesn't call it "The Contra Code".
There were two of them:
Thirty Lives were Up, Up, Down, Down, Left, Right, Left, Right, B, A
Infinite lives were Up, Down, Up, Down, Left, Right, Left, Right, A, B, A, B, Select, Start
I don't know any male in about his mid to late-twenties who doesn't know one of those codes by heart. Go into a bar and ask any guy around that age "The Contra Code" and chances are he'll be able to tell you it.

I am going on the assumption that #3 is "Select, Select, Select" and not "Start, Start, Start" because Google grudgingly coughed up an answer for the former but not the latter: Samurai Showdown
